Our knowledgeable and reliable Support Coordinators are committed to working with and for you to achieve positive outcomes. Your Support Coordinator will take the time to get to know you, your goals, interests and needs to find the right solutions for you. We are based in Craigieburn and provide support to people with a disability throughout Melbourne.

We can provide short term assistance to implement your NDIS plan, including identifying service providers and negotiating Service Agreements.
Support Coordination
We can also support you with longer-term support to build your capacity to navigate the complexity of services and support you need from a range of providers.

Identify the services and service providers that best meet your individual needs
Coordinate service providers and supports, or build your capacity to coordinate your support
Obtain quotes for services and set up Service Agreements with providers
Source suitable accommodation and respite options
Coordinate assessments, reports, and health and medical specialists
Identify solutions for managing risks
Monitor your NDIS plan outcomes and expenditure
Assist you in developing your goals before your NDIS plan review.
